Распределение венечных артерий при врожденных пороках сердца 1. Распределение венечных артерий в сердцах с ВПС с нормальным разворотом конотрункуса с интактной межжелудочковой перегородкой В доступной литературе мы не нашли материала по изучению закономерностей и особенностей отхождения и ветвления ВА при изолированном стенозе легочной артерии с интактной межжелудочковой перегородкой (CJIA с И М Ж П ). В единичных сообщениях описываются аномалии ВА, встречающиеся при данном пороке крайне редко. Нами изучены особенности анатомии центральных отделов венечно артериаль ной системы при СЛА с И М Ж П, а для оценки ишемических изменений в миокарде при данном пороке изучено и периферическое русло (И. И. Беришвили и др., 1990).
Здесь мы ограничимся описанием только тех фрагментов работы, которые касаются анатомии эпикардиальных стволов ВА. В основу материала исследования положены данные изучения центральных отделов венечно артериальной системы 15 больных с Преимущественное большинство сердец (14 из 15, или 93,3%) имело I тип соединения ВА. Средний угол разворота конотрункуса составил 45,2° (разброс вариант от до 62 ). В одном сердце имелась единственная ВА (разворот конотрункуса на 54°). Во всех сердцах с I типом соединения ВА правая В А отходила от 1 го лицевого синуса аорты, а левая ВА от 2 го, делясь на переднюю межжелудочковую и огибающую ветви.
Во всех этих сердцах был сохранен принцип выравнивания комиссур лицевых артериальных клапанов и были соблюдены закономерности отхождения, следования и ветвления ВА, присущие нормальному сердцу.
Как уже отмечалось выше, исключение из группы составил больной с единственной ВА, отходящей от 2 го лицевого синуса аорты с кровоснабжением правой ВА от П М Ж В. При этом правая ВА пересекала подлегочный инфундибулум тотчас ниже клапана легочной артерии. Интересно, что единственная ВА в данном случае не была заподозрена ни при ангиокардиографическом исследовании, ни на операции. На операции диагностировано отхождение П М Ж В от правой ВА.
